Submit this form AS SOON AS you are cut-out of a class, no later than seven (7) days from being cut-out.

BEFORE you submit this form, it is ALWAYS beneficial to have a conversation with your instructor(s) regarding your absences and recommendations on moving forward. Also, this form is not for any single absence; it is for if you have the maximum number of absences for any single class.

Please complete a separate form for each course you are appealing to begin the absence appeal process. This form will be sent to the appropriate Dean or Associate Vice President based on your answers.

Double-check your email address spelling to ensure you receive the decision of your absence appeal.
For academic courses: If you have not been officially dropped/cut out from a class by your instructor, do not submit this form. Speak with your instructor about your absences.
